About every callchains recorded with perf record are filled up
including the internal perfcounter nmi frame:

We want ignore this frame as it's not interesting for
instrumentation. To solve this, we simply ignore every frames
from nmi context.

Change header guards named "ASM_X86__*" to "_ASM_X86_*" since:
a. the double underscore is ugly and pointless.
b. no leading underscore violates namespace constraints.
