Hotplug memory remove notifier for 64-bit powerpc. This gets invoked
by writing to /proc/ppc64/ofdt the string "remove_node " followed by
the firmware device tree pathname of the node that needs to be removed.
In response, this adjusts the sections and removes sysfs entries by
calling __remove_pages(). Then it calls arch-specific code to get rid
of the hardware MMU mappings for the section of memory.

Add a DEBUG config setting which turns on all (most) of the debugging
under platforms/pseries.
To have this take effect we need to remove all the #undef DEBUG's, in
various files. We leave the #undef DEBUG in platforms/pseries/lpar.c,
as this enables debugging printks from the low-level hash table routines,
and tends to make your system unusable. If you want those enabled you
still have to turn them on by hand.
Also some of the RAS code has a DEBUG block which causes a functional
change, so I've keyed this off a different (non-existant) debug #define.
This is only enabled if you have PPC_EARLY_DEBUG enabled also.

Initial patch for reserving memory in early boot, and freeing it
later. If the previous boot had ended with a crash, the reserved
memory would contain a copy of the crashed kernel data.

Implement MSI support via RTAS (RTAS = run-time firmware on pSeries
machines). For now we assumes that if the required RTAS tokens for
MSI are present, then we want to use the RTAS MSI routines.
When RTAS is managing MSIs for us, it will/may enable MSI on devices that
support it by default. This is contrary to the Linux model where a device
is in LSI mode until the driver requests MSIs.
To remedy this we add a pci_irq_fixup call, which disables MSI if they've
been assigned by firmware and the device also supports LSI. Devices that
don't support LSI at all will be left as is, drivers are still expected
to call pci_enable_msi() before using the device.
At the moment there is no pci_irq_fixup on pSeries, so we can just set it
unconditionally. If other platforms use the RTAS MSI backend they'll need
to check that still holds.

During power outages, the UPS notifies the system for a shutdown.
In the current setup, it isn't possible to poweron when power is
restored. This patch fixes the issue by calling the right
ibm,power-off-ups token during such events. It also adds a sysfs
interface so userspace can specify whether or not to power on when
power is restored.

Move all the pseries kexec code into one file, platforms/pseries/kexec.c
Provide helpers for setting up ppc_md.kexec_cpu_down, so that we don't
have to have #ifdef CONFIG_KEXEC in setup.c
Move the initialisation of the ppc_md kexec callbacks into an init routine.
This is well and truly early enough to cause no change in behaviour, we
can't kexec until userspace has given us a kernel to kexec into.

As the first step in consolidating the pseries hotplug cpu code,
create platforms/pseries/hotplug-cpu.c and move rtas_stop_self()
into it. Do the rtas token initialisation in a new initcall, rather
than rtas_initialize().

Add instrumentation for hypervisor calls on pseries. Call statistics
include number of calls, wall time and cpu cycles (if available) and
are made available via debugfs. Instrumentation code is behind the
HCALL_STATS config option and has no impact if not enabled.

The core EEH file is rather large. This patch splits out a self-contained
chunk of it into its own file. This is the chunk that performes the
caching and lookup of pci devices based on the i/o addresses of thier
resoures. This code is almos architecture-independent and could be
used by any system that wanted to find a pci device based only on
the i/o address used by the device.

Various PCI bus errors can be signaled by newer PCI controllers. The
core error recovery routines are architecture dependent. This patch adds
a recovery infrastructure for the PPC64 pSeries systems.

ppc64: EEH Recovery dispatcher thread
This patch adds a mechanism to create recovery threads when an
EEH event is received. Since an EEH freeze state may be detected
within an interrupt context, we need to get out of the interrupt
context before starting recovery. This dispatcher does this in
two steps: first, it uses a workqueue to get out, and then
lanuches a kernel thread, so that the recovery routine can
sleep for exteded periods without upseting the keventd.
A kernel thread is created with each EEH event, rather than
having one long-running daemon started at boot time. This is
because it is anticipated that EEH events will be very rare
(very very rare, ideally) and so its pointless to cluter the
process tables with a daemon that will almost never run.

cell wants them as well as pseries (and chrp can use rtas-proc.c too,
at least in principle). rtas_fw.c is gone, with its bits moved into
rtas_flash.c and rtas.c.

Cell uses the same code as pSeries for flashing the firmware
through rtas, so the implementation should not be part of
platforms/pseries.

This patch moves the XICS interrupt controller code into the
platforms/pseries directory, since it only appears on pSeries
machines. If it ever appears on some other machine we can move it to
sysdev, although xics.c itself will need a bunch of changes in that
case to remove pSeries specific assumptions.

This splits arch/ppc64/kernel/rtas.c into arch/powerpc/kernel/rtas.c,
which contains generic RTAS functions useful on any CHRP platform,
and arch/powerpc/platforms/pseries/rtas-fw.[ch], which contain
some pSeries-specific firmware flashing bits. The parts of rtas.c
that are to do with pSeries-specific error logging are protected
by a new CONFIG_RTAS_ERROR_LOGGING symbol. The inclusion of rtas.o
is controlled by the CONFIG_PPC_RTAS symbol, and the relevant
platforms select that.
