8a13c4f972 [POWERPC] Use PAGE_OFFSET to tell if an address is user/kernel in SW TLB handlers
Move to using PAGE_OFFSET instead of TASK_SIZE or KERNELBASE value on
6xx/40x/44x/fsl-booke to determine if the faulting address is a kernel or
user space address. This mimics how the macro is_kernel_addr() works.

cdec12aebe [POWERPC] Make clockevents work on PPC601 processors
In testing the new clocksource and clockevent code on a PPC601
processor, I discovered that the clockevent multiplier value for the
decrementer clockevent was overflowing. Because the RTCL register in
the 601 effectively counts at 1GHz (it doesn't actually, but it
increases by 128 every 128ns), and the shift value was 32, that meant
the multiplier value had to be 2^32, which won't fit in an unsigned
long on 32-bit. The same problem would arise on any platform where
the timebase frequency was 1GHz or more (not that we actually have any
such machines today).

This fixes it by reducing the shift value to 16. Doing the
calculations with a resolution of 2^-16 nanoseconds (15 femtoseconds)
should be quite adequate. :)

d968014b72 [POWERPC] Prevent decrementer clockevents from firing early
On old powermacs, we sometimes set the decrementer to 1 in order to
trigger a decrementer interrupt, which we use to handle an interrupt
that was pending at the time when it was re-enabled. This was causing
the decrementer clock event device to call the event function for the
next event early, which was causing problems when high-res timers were
not enabled.

This fixes the problem by recording the timebase value at which the
next event should occur, and checking the current timebase against the
recorded value in timer_interrupt. If it isn't time for the next
event, it just reprograms the decrementer and returns.

This also subtracts 1 from the value stored into the decrementer,
which is appropriate because the decrementer interrupts on the
transition from 0 to -1, not when the decrementer reaches 0.

87a72f9e17 [POWERPC] Fix performance monitor on machines with logical PVR
Some IBM machines supply a "logical" PVR (processor version register)
value in the device tree in the cpu nodes rather than the real PVR.
This is used for instance to indicate that the processors in a POWER6
partition have been configured by the hypervisor to run in POWER5+
mode rather than POWER6 mode. To cope with this, we call identify_cpu
a second time with the logical PVR value (the first call is with the
real PVR value in the very early setup code).

However, POWER5+ machines can also supply a logical PVR value, and use
the same value (the value that indicates a v2.04 architecture
compliant processor). This causes problems for code that uses the
performance monitor (such as oprofile), because the PMU registers are
different in POWER6 (even in POWER5+ mode) from the real POWER5+.

This change works around this problem by taking out the PMU
information from the cputable entries for the logical PVR values, and
changing identify_cpu so that the second call to it won't overwrite
the PMU information that was established by the first call (the one
with the real PVR), but does update the other fields. Specifically,
if the cputable entry for the logical PVR value has num_pmcs == 0,
none of the PMU-related fields get used.

So that we can create a mixed cputable entry, we now make cur_cpu_spec
point to a single static struct cpu_spec, and copy stuff from
cpu_specs[i] into it. This has the side-effect that we can now make
cpu_specs[] be initdata.

Ultimately it would be good to move the PMU-related fields out to a
separate structure, pointed to by the cputable entries, and change
identify_cpu so that it saves the PMU info pointer, copies the whole
structure, and restores the PMU info pointer, rather than identify_cpu
having to list all the fields that are *not* PMU-related.

84fdde5af1 [POWERPC] Use cache-inhibited large page bit from firmware
Discussions with firmware architects have confirmed that the bit in
the ibm,pa-features property that indicates support for
cache-inhibited large (>= 64kB) page mappings does in fact mean that
the hypervisor allows 64kB mappings to I/O devices.

Thus we can now enable the code that tests that bit and sets our

Previously, Soft_emulate_8xx was called with no implementation, resulting in
build failures whenever building 8xx without math emulation. The
implementation is copied from arch/ppc to resolve this issue.

However, this sort of minimal emulation is not a very good idea other than
for compatibility with existing userspaces, as it's less efficient than
soft-float and can mislead users into believing they have soft-float. Thus,
it is made a configurable option, off by default.

26f571d7c9 [PPC] Use cpu setup routines from cpu_setup_44x.S for ARCH=ppc
Commit 8112753bb2 made 44x in
ARCH=powerpc builds use cpu setup routines in cpu_setup_44x.S,

the ARCH=ppc builds fail with undefined symbols (since both use

This fixes it by including cpu_setup_44x.S in the ARCH=ppc builds,

048c8bc90e [POWERPC] ppc64: support CONFIG_DEBUG_PREEMPT
Add CONFIG_DEBUG_PREEMPT support to ppc64: it was useful for testing
get_paca() preemption. Cheat a little, just use debug_smp_processor_id()
in the debug version of get_paca(): it contains all the right checks and
reporting, though get_paca() doesn't really use smp_processor_id().

Use local_paca for what might have been called __raw_get_paca().
Silence harmless warnings from io.h and lparcfg.c with local_paca -
it is okay for iseries_lparcfg_data to be referencing shared_proc
with preemption enabled: all cpus should show the same value for
shared_proc.

Why do other architectures need TRACE_IRQFLAGS_SUPPORT for DEBUG_PREEMPT?

The fix tries to use the smallest scope CONFIG_* symbols that will fix
the build problem. In this case <linux/ide.h> needs to be included
only if IDE=y or IDE=m were selected. Also, ppc_ide_md is needed only
if BLK_DEV_IDE=y or BLK_DEV_IDE=m

Moved the EXPORT_SYMBOL(ppc_ide_md) from ppc_ksysms.c next to its
declaration in setup_32.c which made <linux/ide.h> not needed. With
<linux/ide.h> gone from ppc_ksyms.c, <asm/cacheflush.h> is needed to
address the following warnings and errors:

8fd7675c09 [POWERPC] Avoid pointless WARN_ON(irqs_disabled()) from panic codepath
> ------------[ cut here ]------------
> Badness at arch/powerpc/kernel/smp.c:202

comes when smp_call_function_map() has been called with irqs disabled,
which is illegal. However, there is a special case, the panic() codepath,
when we do not want to warn about this -- warning at that time is pointless
anyway, and only serves to scroll away the *real* cause of the panic and
distracts from the real bug.

* So let's extract the WARN_ON() from smp_call_function_map() into all its
  callers -- smp_call_function() and smp_call_function_single()

* Also, introduce another caller of smp_call_function_map(), namely
__smp_call_function() (and make smp_call_function() a wrapper over this)
which does *not* warn about disabled irqs

* Use this __smp_call_function() from the panic codepath's smp_send_stop()

We also end having to move code of smp_send_stop() below the definition
of __smp_call_function().

c6d4267ece [POWERPC] Handle alignment faults on new FP load/store instructions
This adds code to handle alignment traps generated by the following
new floating-point load/store instructions, by emulating the
instruction in the kernel (as is done for other instructions that
generate alignment traps):

lfiwax	load floating-point as integer word algebraic indexed
stfiwx	store floating-point as integer word indexed
lfdp	load floating-point double pair
lfdpx	load floating-point double pair indexed
stfdp	store floating-point double pair
stfdpx	store floating-point double pair indexed

All these except stfiwx are new in POWER6.

lfdp/lfdpx/stfdp/stfdpx load and store 16 bytes of memory into an
even/odd FP register pair. In little-endian mode each 8-byte value is
byte-reversed separately (i.e. not as a 16-byte unit). lfiwax/stfiwx
load or store the lower 4 bytes of a floating-point register from/to
memory; lfiwax sets the upper 4 bytes of the FP register to the sign
extension of the value loaded.

55a910a81d [POWERPC] Fix for assembler -g
ppc64 does the unusual thing of using #include on a compiler-generated

aa1cf632bd [POWERPC] Fix small race in 44x tlbie function
The 440 family of processors don't have a tlbie instruction. So, we
implement TLB invalidates by explicitly searching the TLB with tlbsx.,
then clobbering the relevant entry, if any. Unfortunately the PID for
the search needs to be stored in the MMUCR register, which is also
used by the TLB miss handler. Interrupts were enabled in _tlbie(), so
an interrupt between loading the MMUCR and the tlbsx could cause
incorrect search results, and thus a failure to invalide TLB entries
which needed to be invalidated.

This fixes the problem in both arch/ppc and arch/powerpc by inhibiting
interrupts (even critical and debug interrupts) across the relevant
instructions.

8f2ea1fd3f [POWERPC] Fix initialization and usage of dma_mask
powerpc has a couple of bugs in the usage of dma_masks that tend to
break when drivers explicitly try to set a 32-bit mask for example.

First, the code that generates the pci devices from the OF device-tree
doesn't initialize the mask properly, then our implementation of
set_dma_mask() was trying to validate the -previous- mask value, not the
one passed in as an argument.

This fixes these problems.

This patch uses the newly added functions for testing if an address is

77926826f3 Revert "[POWERPC] Don't complain if size-cells == 0 in prom_parse()"
This reverts commit fd6e9d3945.

Having #size-cells == 0 in a node indicates that things under the
node aren't directly accessible, and therefore we shouldn't try to
translate addresses for devices under the node into CPU physical
addresses.

Some drivers, such as the nvram driver for powermacs, rely on
of_address_to_resource failing if they are called for a node
representing a device whose resources aren't directly accessible
by the CPU. These drivers were broken by commit fd6e9d39,
resulting in the "Lombard" powerbook hanging early in the boot
process.

1474855d08 [CELL] oprofile: add support to OProfile for profiling CELL BE SPUs
From: Maynard Johnson <mpjohn@us.ibm.com>

This patch updates the existing arch/powerpc/oprofile/op_model_cell.c
to add in the SPU profiling capabilities. In addition, a 'cell' subdirectory
was added to arch/powerpc/oprofile to hold Cell-specific SPU profiling code.
Exports spu_set_profile_private_kref and spu_get_profile_private_kref which
are used by OProfile to store private profile information in spufs data
structures.

Also incorporated several fixes from other patches (rrn). Check pointer
returned from kzalloc. Eliminated unnecessary cast. Better error
handling and cleanup in the related area. 64-bit unsigned long parameter
was being demoted to 32-bit unsigned int and eventually promoted back to

5fb7dc37dc define new percpu interface for shared data
per cpu data section contains two types of data. One set which is
exclusively accessed by the local cpu and the other set which is per cpu,
but also shared by remote cpus. In the current kernel, these two sets are
not clearely separated out. This can potentially cause the same data
cacheline shared between the two sets of data, which will result in
unnecessary bouncing of the cacheline between cpus.

One way to fix the problem is to cacheline align the remotely accessed per
cpu data, both at the beginning and at the end. Because of the padding at
both ends, this will likely cause some memory wastage and also the
interface to achieve this is not clean.

This patch:

Moves the remotely accessed per cpu data (which is currently marked
as ____cacheline_aligned_in_smp) into a different section, where all the data
elements are cacheline aligned. And as such, this differentiates the local
only data and remotely accessed data cleanly.

3d7e33825d jprobes: make jprobes a little safer for users
I realise jprobes are a razor-blades-included type of interface, but that
doesn't mean we can't try and make them safer to use. This guy I know once
wrote code like this:

struct jprobe jp = { .kp.symbol_name = "foo", .entry = "jprobe_foo" };

And then his kernel exploded. Oops.

This patch adds an arch hook, arch_deref_entry_point() (I don't like it
either) which takes the void * in a struct jprobe, and gives back the text
address that it represents.

We can then use that in register_jprobe() to check that the entry point we're
passed is actually in the kernel text, rather than just some random value.

97ac73506c sys_fallocate() implementation on i386, x86_64 and powerpc
fallocate() is a new system call being proposed here which will allow
applications to preallocate space to any file(s) in a file system.
Each file system implementation that wants to use this feature will need
to support an inode operation called ->fallocate().
Applications can use this feature to avoid fragmentation to certain
level and thus get faster access speed. With preallocation, applications
also get a guarantee of space for particular file(s) - even if later the
the system becomes full.

Currently, glibc provides an interface called posix_fallocate() which
can be used for similar cause. Though this has the advantage of working
on all file systems, but it is quite slow (since it writes zeroes to
each block that has to be preallocated). Without a doubt, file systems
can do this more efficiently within the kernel, by implementing
the proposed fallocate() system call. It is expected that
posix_fallocate() will be modified to call this new system call first
and incase the kernel/filesystem does not implement it, it should fall
back to the current implementation of writing zeroes to the new blocks.
ToDos:
1. Implementation on other architectures (other than i386, x86_64,
and ppc). Patches for s390(x) and ia64 are already available from
previous posts, but it was decided that they should be added later
once fallocate is in the mainline. Hence not including those patches
   in this take.
2. Changes to glibc,
   a) to support fallocate() system call
   b) to make posix_fallocate() and posix_fallocate64() call fallocate()

608e261968 generic bug: use show_regs() instead of dump_stack()
The current generic bug implementation has a call to dump_stack() in case a
WARN_ON(whatever) gets hit. Since report_bug(), which calls dump_stack(),
gets called from an exception handler we can do better: just pass the
pt_regs structure to report_bug() and pass it to show_regs() in case of a
warning. This will give more debug informations like register contents,
etc... In addition this avoids some pointless lines that dump_stack()
emits, since it includes a stack backtrace of the exception handler which
is of no interest in case of a warning. E.g. on s390 the following lines
are currently always present in a stack backtrace if dump_stack() gets
called from report_bug():

 [<000000000001517a>] show_trace+0x92/0xe8)
 [<0000000000015270>] show_stack+0xa0/0xd0

fc9069fe56 [POWERPC] Modify sched_clock() to make CONFIG_PRINTK_TIME more sane
When booting a current kernel with CONFIG_PRINTK_TIME enabled you'll

[3712914.436297] Console: colour dummy device 80x25

This cause by the initialisation of tb_to_ns_scale in time_init(), suddenly the
multiplication in sched_clock() now does something :). This patch modifies
sched_clock() to report the offset since the machine booted so the same
printk's now look like:

[ 0.000000] time_init: decrementer frequency = 188.044000 MHz
[ 0.000000] time_init: processor frequency = 1504.352000 MHz
[ 0.000135] Console: colour dummy device 80x25

edd5cd4a94 Introduce fixed sys_sync_file_range2() syscall, implement on PowerPC and ARM
Not all the world is an i386. Many architectures need 64-bit arguments to be
aligned in suitable pairs of registers, and the original
sys_sync_file_range(int, loff_t, loff_t, int) was therefore wasting an
argument register for padding after the first integer. Since we don't
normally have more than 6 arguments for system calls, that left no room for
the final argument on some architectures.

Fix this by introducing sys_sync_file_range2(int, int, loff_t, loff_t) which
all fits nicely. In fact, ARM already had that, but called it
sys_arm_sync_file_range. Move it to fs/sync.c and rename it, then implement
the needed compatibility routine. And stop the missing syscall check from
bitching about the absence of sys_sync_file_range() if we've implemented
sys_sync_file_range2() instead.

Tested on PPC32 and with 32-bit and 64-bit userspace on PPC64.

ae62fbb5f1 [POWERPC] Fix subtle FP state corruption bug in signal return on SMP
This fixes a bug which can cause corruption of the floating-point state
on return from a signal handler. If we have a signal handler that has
used the floating-point registers, and it happens to context-switch to
another task while copying the interrupted floating-point state from the
user stack into the thread struct (e.g. because of a page fault, or
because it gets preempted), the context switch code will think that the
FP registers contain valid FP state that needs to be copied into the
thread_struct, and will thus overwrite the values that the signal return
code has put into the thread_struct.

This can occur because we clear the MSR bits that indicate the presence
of valid FP state after copying the state into the thread_struct. To fix
this we just move the clearing of the MSR bits to before the copy. A
similar potential problem also occurs with the Altivec state, and this
fixes that in the same way.

df211c8a47 [POWERPC] Remove spinlock from struct cpu_purr_data
cpu_purr_data is a per-cpu array used to account for stolen time on
partitioned systems. It used to be the case that cpus accessed each
others' cpu_purr_data, so each entry was protected by a spinlock.

However, the code was reworked ("Simplify stolen time calculation")
with the result that each cpu accesses its own cpu_purr_data and not
those of other cpus. This means we can get rid of the spinlock as
long as we're careful to disable interrupts when accessing
cpu_purr_data in process context.

With this, when kexec-ing, we copy the code and start the slaves on

The kernel doesn't know exactly which slaves are spinning in
kexec_wait. This allows us to pass more than max-cpus to the
next kernel. But it also means that we might leave some behind.

Moving the code here means they have the time it takes us to
clear the hash table to wake up and move on. Moving the code

4508dc21fe [POWERPC] Merge CPU features pertaining to icache coherency
Currently the powerpc kernel has a 64-bit only feature,
COHERENT_ICACHE used for those CPUS which maintain icache/dcache
coherency in hardware (POWER5, essentially). It also has a feature,
SPLIT_ID_CACHE, which is used on CPUs which have separate i and
d-caches, which is to say everything except 601 and Freescale E200.

In nearly all the places we check the SPLIT_ID_CACHE, what we actually
care about is whether the i and d-caches are coherent (which they will
be, trivially, if they're the same cache).

This tries to clarify the situation a little. The COHERENT_ICACHE
feature becomes availble on 32-bit and is set for all CPUs where i and
d-cache are effectively coherent, whether this is due to special logic
(POWER5) or because they're unified. We check this, instead of
SPLIT_ID_CACHE nearly everywhere.

The SPLIT_ID_CACHE feature itself is replaced by a UNIFIED_ID_CACHE
feature with reversed sense, set only on 601 and Freescale E200. In
the two places (one Freescale BookE specific) where we really care
whether it's a unified cache, not whether they're coherent, we check
this feature. The CPUs with unified cache are so few, we could
consider replacing this feature bit with explicit checks against the
PVR.

This will make unifying the 32-bit and 64-bit cache flush code a
little more straightforward.

d8c391a559 [POWERPC] Donate idle CPU cycles on dedicated partitions
A Power6 can give up CPU cycles on a dedicated CPU (as opposed to a
shared CPU) to other shared processors if the administrator asks for it
(via the HMC).

This enables that to work properly on P6.

This just involves setting a bit in the CAS structure as well as the
VPA. To donate cycles, a CPU has to have all SMT threads idle and
have the donate bit set in the VPA. Then call H_CEDE.

The reason why shared processors just aren't used is because dedicated
CPUs are guaranteed an actual processor, yet the system is still able to
increase the capacity of the shared CPU pool.

Also rename the VPA's cpuctls_task_attrs field to a more accurate name.

e17666ba48 [POWERPC] ptrace updates & new, better requests
The powerpc ptrace interface is dodgy at best. We have defined our
"own" versions of GETREGS/SETREGS/GETFPREGS/SETFPREGS that strangely
take arguments in reverse order from other archs (in addition to having
different request numbers) and have subtle issue, like not accessing
all of the registers in their respective categories.

This patch moves the implementation of those to a separate function
in order to facilitate their deprecation in the future, and provides
new ptrace requests that mirror the x86 and sparc ones and use the
same numbers:

PTRACE_GETREGS : returns an entire pt_regs (the whole thing,
not only the 32 GPRs, though that doesn't
include the FPRs etc... There's a compat version
for 32 bits that returns a 32 bits compatible
                       pt_regs (44 uints)

PTRACE_SETREGS : sets an entire pt_regs (the whole thing,
not only the 32 GPRs, though that doesn't
include the FPRs etc... Some registers cannot be
written to and will just be dropped, this is the
same as with POKEUSR, that is anything above MQ
on 32 bits and CCR on 64 bits. There is a compat
                       version as well.

PTRACE_GETFPREGS : returns all the FP registers -including- the FPSCR
that is 33 doubles (regardless of 32/64 bits)

PTRACE_SETFPREGS : sets all the FP registers -including- the FPSCR
that is 33 doubles (regardless of 32/64 bits)

And two that only exist on 64 bits kernels:

PTRACE_GETREGS64 : Same as PTRACE_GETREGS, except there is no compat
function, a 32 bits process will obtain the full 64
                       bits registers

PTRACE_SETREGS64 : Same as PTRACE_SETREGS, except there is no compat
function, a 32 bits process will set the full 64
                       bits registers

The two later ones makes things easier to have a 32 bits debugger on a
64 bits program (or on a 32 bits program that uses the full 64 bits of
the GPRs, which is possible though has issues that will be fixed in a
later patch).

Finally, while at it, the patch removes a whole bunch of code duplication
between ptrace32.c and ptrace.c, in large part by having the former call
into the later for all requests that don't need any special "compat"
treatment.

3d5134ee83 [POWERPC] Rewrite IO allocation & mapping on powerpc64
This rewrites pretty much from scratch the handling of MMIO and PIO
space allocations on powerpc64. The main goals are:

 - Get rid of imalloc and use more common code where possible
 - Simplify the current mess so that PIO space is allocated and
mapped in a single place for PCI bridges
 - Handle allocation constraints of PIO for all bridges including
hot plugged ones within the 2GB space reserved for IO ports,
so that devices on hotplugged busses will now work with drivers
that assume IO ports fit in an int.
 - Cleanup and separate tracking of the ISA space in the reserved
low 64K of IO space. No ISA -> Nothing mapped there.

I booted a cell blade with IDE on PIO and MMIO and a dual G5 so
far, that's it :-)

With this patch, all allocations are done using the code in
mm/vmalloc.c, though we use the low level __get_vm_area with
explicit start/stop constraints in order to manage separate
areas for vmalloc/vmap, ioremap, and PCI IOs.

This greatly simplifies a lot of things, as you can see in the
diffstat of that patch :-)

A new pair of functions pcibios_map/unmap_io_space() now replace
all of the previous code that used to manipulate PCI IOs space.
The allocation is done at mapping time, which is now called from
scan_phb's, just before the devices are probed (instead of after,
which is by itself a bug fix). The only other caller is the PCI
hotplug code for hot adding PCI-PCI bridges (slots).

imalloc is gone, as is the "sub-allocation" thing, but I do beleive
that hotplug should still work in the sense that the space allocation
is always done by the PHB, but if you unmap a child bus of this PHB
(which seems to be possible), then the code should properly tear
down all the HPTE mappings for that area of the PHB allocated IO space.

I now always reserve the first 64K of IO space for the bridge with
the ISA bus on it. I have moved the code for tracking ISA in a separate
file which should also make it smarter if we ever are capable of
hot unplugging or re-plugging an ISA bridge.

This should have a side effect on platforms like powermac where VGA IOs
will no longer work. This is done on purpose though as they would have
worked semi-randomly before. The idea at this point is to isolate drivers
that might need to access those and fix them by providing a proper
function to obtain an offset to the legacy IOs of a given bus.

e147ec8f18 [POWERPC] Simplify smp_space_timers
Greatly simplify the function smp_space_timers.

The stolen time calculation (per comment within the code) doesn't need the
half-jiffy stagger any more. There isn't an issue with bouncing off global

However, the last_jiffy value still needs to be set. This removes the
extra stagger logic, and just sets the values.

This change should benefit applications that rely on barrier

e1fa2e136f powerpc: fixup hard_irq_disable semantics

function that can be used by any code to enforce that IRQs are fully disabled,

The difference with the __ versions is that it will update some per-processor
fields so that the kernel keeps track and properly re-enables them in the next

This prepares powerpc for my next patch that introduces hard_irq_disable()

This finally renames the thread_info field in task structure to stack, so that

8bb7844286 Add suspend-related notifications for CPU hotplug
Since nonboot CPUs are now disabled after tasks and devices have been
frozen and the CPU hotplug infrastructure is used for this purpose, we need
special CPU hotplug notifications that will help the CPU-hotplug-aware
subsystems distinguish normal CPU hotplug events from CPU hotplug events
related to a system-wide suspend or resume operation in progress. This
patch introduces such notifications and causes them to be used during
suspend and resume transitions. It also changes all of the
CPU-hotplug-aware subsystems to take these notifications into consideration
(for now they are handled in the same way as the corresponding "normal"
ones).

d0f13e3c20 [POWERPC] Introduce address space "slices"
The basic issue is to be able to do what hugetlbfs does but with
different page sizes for some other special filesystems; more
specifically, my need is:

 - Huge pages

 - SPE local store mappings using 64K pages on a 4K base page size
kernel on Cell

 - Some special 4K segments in 64K-page kernels for mapping a dodgy
type of powerpc-specific infiniband hardware that requires 4K MMU
mappings for various reasons I won't explain here.

The main issues are:

 - To maintain/keep track of the page size per "segment" (as we can
only have one page size per segment on powerpc, which are 256MB
divisions of the address space).

 - To make sure special mappings stay within their allotted
"segments" (including MAP_FIXED crap)

 - To make sure everybody else doesn't mmap/brk/grow_stack into a
"segment" that is used for a special mapping

Some of the necessary mechanisms to handle that were present in the
hugetlbfs code, but mostly in ways not suitable for anything else.

The patch relies on some changes to the generic get_unmapped_area()
that just got merged. It still hijacks hugetlb callbacks here or
there as the generic code hasn't been entirely cleaned up yet but
that shouldn't be a problem.

So what is a slice ? Well, I re-used the mechanism used formerly by our
hugetlbfs implementation which divides the address space in
"meta-segments" which I called "slices". The division is done using
256MB slices below 4G, and 1T slices above. Thus the address space is
divided currently into 16 "low" slices and 16 "high" slices. (Special
case: high slice 0 is the area between 4G and 1T).

Doing so simplifies significantly the tracking of segments and avoids
having to keep track of all the 256MB segments in the address space.

While I used the "concepts" of hugetlbfs, I mostly re-implemented
everything in a more generic way and "ported" hugetlbfs to it.

Slices can have an associated page size, which is encoded in the mmu
context and used by the SLB miss handler to set the segment sizes. The
hash code currently doesn't care, it has a specific check for hugepages,
though I might add a mechanism to provide per-slice hash mapping
functions in the future.

The slice code provide a pair of "generic" get_unmapped_area() (bottomup
and topdown) functions that should work with any slice size. There is
some trickiness here so I would appreciate people to have a look at the
implementation of these and let me know if I got something wrong.
