As the STV0900 features two demodulation paths in one chip there is
some information used by both instances of the driver when used in
dual mode. This information is now shared in an internal structure
referenced by I2C adapter and address.
Do initialisation of the demodulator only once when used in dual mode.
Moved global mutex demod_lock to internal structure.
Moved dev_ver and mclk to internal structure.
Removed unused tuner_refclk from stv090x_state.

In automatic S/S2 detection mode, locking of a DVB-S transponder could
fail when coming from a DVB-S2 transponder. This change fixes the issue
by first disabling DVB-S and DVB-S2 mode before enabling it again.

22-kHz tone can be driven in two ways:
1. LNBH24 can produce 22kHz continuous tone when TEN=1 ( 22 KHz tone output is
always activated ).
2. LNBH24 can reproduce 22kHz tone timings from DSQIN or EXTM pin's when TEN=0.
From LNBH24 datasheet:
"In order to improve design flexibility an external tone input pin is
available (EXTM). The EXTM is a Logic input pin which activates the 22 kHz
tone output, on the VoTX pin, by using the LNBH24 integrated tone generator
(similar to the DSQIN pin function). In fact, the output tone waveform
characteristics will always be internally controlled by the LNBH24 tone
generator and the EXTM signal will be used as a timing control for DiSEqC tone
data encoding on the VoTX output."
In NetUP Dual DVB-S2-CI card 22kHz tone timings on EXTM pin produced by
STV0900 demod:
.set_tone = stv0900_set_tone
redefine to "set_tone = lnbp21_set_tone" is not correct for "NetUP Dual
DVB-S2-CI card".

I'm trying to fix it on the GCC side (PR43007), but the module is
quite stupid in using ULL constants to operate on u32 values:

On particularly weak signals, changing register 0xab after setting the
interleave mode will cause the FEC lock to get lost (while still holding the
EQ lock). So remove the write entirely, which seems to have had no adverse
effect in either of my ClearQAM environments (and in particular resolves the
customer's reported issue).
Also flip around the order of the amhum optimization and the interleave setup
to be consistent with the windows driver, which has the side-benefit of doing
them both in the same call (since the amhum optimization can only be done once
the interleave setup).

The sh51409 driver was only doing the QAM optimization a single time, and it
would only occur if you received a lock instantaneously after the tuning
request. Restructure the code so that the optimization occurs once you reach
a signal lock.
Note that this depends on the caller polling for status, but we don't have
much choice at this point without an independent thread monitoring the lock
status. Also, at this point pretty much every application polls for status
lock after doing the tune, so the likelihood of the optimization not occurring
in the real world is pretty low.
The state machine has also been reworked such that setting the interleave mode
is now a dependency of doing the QAM optimization. Before both were mutually
exclusive, which was not consistent with the Windows driver. We now have a
single state machine that controls both.
The changes as-is are only enabled for the HVR-1600. Once the changes are
tested with some of the other boards, this change should be made generic and
the "_legacy" functions should be removed.

Perform some optimization of the register configuration based on a trace
of the HVR-1600 Windows i2c traffic (and consultation with Steven Toth).
Note that some of these values may be able to be moved into the common driver,
but I am holding off on that until they can be tested with other boards.

The status is HAS_LOCK only if the demod is able to decode the TPS. Sometimes, there is a TPS data lock, even if the demod is not able to decode it (ex: no RF signal).

Fixing kernel oops when driver attemps to load xc2028 firmware.
Note by djh: the patch contribute by Martin is a port of a fix I made during
the PCTV 340e development. It's a temporary workaround that fixes a regression
(an OOPS condition) and the real fix should be in the code that manages the
i2c master on the dib7000p. But this fix does address the immmediate
regression and should be merged upstream until we do a cleaner fix.

Attached is a patch which fixes tuning to low frequency channels with
stb0899+tda8261 cards like the KNC TV-Station DVB-S2.
The cause of the issue was a broken if construct, which should have been
an if/else if, so that the setting for the lowest matching frequency is
applied.
Without this patch for example tuning to "arte" on Astra 19.2, 10744MHz
SR22000 failed most times and when it failed the communication between
driver and tda8261 was completely broken.
This problem disappears with the attached patch.

In automatic mode every stop event on SDA line ends repetition.
However, in NetUP Dual card on the same i2c bus we have several devices.
If someone using both adapters to lock simultaneously or working with CAM interface
during lock procedure, it lead to end repetition prematurely quite often.
Set stv0900 i2c repeater to manual mode prevents such situation.

No datasheet, data take from code in flexcop driver. That code rounded
down the divisor rather than rounding to nearest, which was probably not
intentional and the dvb-pll code will round to nearest.

Tuner parameters taken from flexcop driver. This PLL has a 17 bit divisor
while the dvb-pll driver is designed for 15 bit divisors. It's not a
problem as 15 bits is enough for the tuner's entire range. But if a larger
range was wanted, it could be done by adding additional bands with the
extra divisor bits appearing as band switch bits.

This patch changes most frontend drivers to allocate their state structure via
kzalloc and not kmalloc. This is done to properly initialize the
embedded "struct dvb_frontend frontend" field, that they all have.
The visible effect of this struct being uninitalized is, that the member "id"
that is used to set the name of kernel thread is totally random.
Some board drivers (for example cx88-dvb) set this "id" via
videobuf_dvb_alloc_frontend but most do not.
So I at least get random id values for saa7134, flexcop and ttpci based cards.
It looks like this in dmesg:
DVB: registering adapter 1 frontend -10551321 (ST STV0299 DVB-S)
The related kernel thread then also gets a strange name
like "kdvb-ad-1-fe--1".

Fixes stack corruption bug present in dump_regs function of zl10353 and
qt1010 drivers: the buffer buf was one byte smaller than required -
there are 4 chars for address prefix, 16 * 3 chars for dump of 16 eeprom
bytes per line and 1 byte for zero ending the string required, i.e. 53
bytes, but only 52 were provided.
The one byte missing in stack based buffer buf can cause stack
corruption possibly leading to kernel oops, as discovered originally
with af9015 driver (af9015: fix stack corruption bug).

Request demodulator auto-detect transmission parameters in case of
garbage parameters provided by application for compatibility.
That's needed at least for MPlayer compatibility currently.

loaded for a given chip version. This would cause the optimization in
tuning not to be applied and thus a failed expectation, in tuning speed
increment. The patch swaps the tables in use. It also fixes a possible
one in a million condition where state->dev_ver implies an older Cut
(Cut < 2.0, eventhough the driver doesn't attach to any Cut older than
2.0) or even negative (due to a bad I2C bus master driver) for the card
combination.

Currently, DVB-T is broken and this fixes it.
The PVRUSB2 has an odd I2C bus configuration where opening the i2c gate
on the digital and analog demod causes the tuner to fail. This needs
to be protected against for the PVRUSB2.

Use the physical device rather than the i2c adapter as the reference
device when loading firmwares. This will prevent the sysfs name
collision with i2c-dev that has been reported many times.
I may have missed other drivers which need the same fix.
