fb11137af8 b43: Split PHY alloc and init 
This splits the PHY allocation from the PHY init.
This is needed in order to properly support Analog handling.

3d0da75164 b43: Move code from nphy.* to phy_n.* 
Sourcecode files for PHY code are named by phy_XXX.{c,h}
where XXX is the PHY type.
Move the N-PHY code to match the other files.
