Peter Oberparleiter 4e8e56c671 [S390] cio: Extend adapter interrupt interface.
From: Cornelia Huck <cornelia.huck@de.ibm.com>

Change the adapter interrupt interface in order to allow multiple
adapter interrupt handlers to be registered. Indicators are now
allocated by cio instead of the device driver.
