a0d471cb0b Improve FT8 SNR estimates in two ways: 
(i) SNR no longer saturates at +20 dB
(ii) a large signal in the passband no longer causes the
SNR of weaker signals to be biased low.

d1530b0c19 Rename ft8sim files as follows: 
ft8sim_gfsk.f90 => ft8sim.f90
ft8sim.f90 => ft8sim_fsk.f90

This change reflects the fact that the GFSK waveform is now used in WSJT-X
for ft8 and ft4.

Log FT8 DXpedition mode dupe contacts 
Not logging dupes seems to cuase  problems with QSL matching where the
Hound  fails  to   log  when  an  RR73  message   is  not  succesfully
received. By logging dupes a later retry  by a Hound to complete a QSO
will be recorded in the Fox's log.

Use call, mode, and frequency from Log QSO for worked before records 
The current frequency,  mode and, call were incorrectly  being used to
create a new  worked before record from a logged  QSO. This meant that
band changes  etc. made  before clicking  "Ok" to log  a QSO  would be
erroneously attributed to the worked before records.

Allow double-click of decode with unmapped hash code 
This allows  the DX  call and  messages to  be populated  from decodes
where the first callsign is an unresolved hash code <...>.

Add option to highlight un-worked grid fields 
This  is a  check box  option  in "Settings->Colors"  rather than  new
    highlighting types so un-worked  field highlighting and un-worked grid
    square highlighting are mutually exclusive. The check box state can be
    changed at  any time, no  log rescanning is necessary,  and subsequent
    decoded message highlighting will be according to the check box state.

Fix up message parsing regexp to handle 4 character directional CQs 
This defect was causing new grids not  to be highlighted when 1, 3, or
4 directional  CQ words  were used,  as allowed  by the  77-bit source
encoding.
